Applicant Eligibility:

 Must be resident of the State of Florida.

 Must be between the ages of birth and 21 years old.

 Must have a condition necessitating physical, speech or occupational therapy.

 Must have financial need for the services.

 Must demonstrate that equipment could not be obtained first through insurance (Medicaid, private insurance, APD agency for persons with disabilities, school district, United Way) and or that it has been denied or is not available from any of these sources.

 Must provide a statement from attending physician or therapist that this equipment will increase the applicant’s life condition, giving better mobility, position, communication, vision, speech and/or hearing.
